Great opportunity within a great hospital. Responsibilities include providing oversight and direction to care coordination, utilization review, social services, and discharge planning. Additionally you will
work closely with medical staff and hospital leadership to identify patient care trends & issues and develop care management programs, procedures, and clinical pathways that ensure efficient use of medical resources and provide optimal cost-effective, quality care.
